Pre-Testing Requirements

Serum samples should be taken from patients receiving TNF Biologics for at least 10 weeks.

Samples should be taken at drug trough, within the 24 hours prior to a dose.

Please indicate which TNF biologic drug the patient is receiving.

Interpretation

The tests performed measure concentration of free anti-TNF drug in diluted serum samples.  The measured drug concentration may be used to adjust dose and interval to optimise clinical response.

Trough concentrations below 7mg/L using this assay have been associated with reduced disease control in inflammatory bowel disease.

Many patients lose response to TNF biologics over time.  Immunogenicity to the biologic drug is one commonly identified factor.  Low or undetectable TNF biologic trough concentrations may indicate the presence of anti-drug antibodies (ADA).  High drug concentrations will mask the presence of ADA. We measure presence of ADA when the drug concentration is < 2mg/L.

Transient antibodies to drug have been reported.

Detection of ADA may precede the onset of loss of response by several months
